Forms for Parents
Remember that you do NOT need to attend registration in person if you do the online portion and also provide filled out form hard copies to the appropriate building offices before the first day of school. Forms can also be mailed if preferred.
SOME of the forms below were mailed to you (certificate of dental screening, certificate of visual screening, Iowa application for free and reduced meals). If you need a replacement copy of these forms they are available below to download and print.
TWO forms at the bottom were NOT mailed to you (Request for Administering Medication in School, Proof of Residency Form). If you need hard copies of these forms please download and print them.
**REQUIRED for Kindergarten & 9th grade students.
-Print this form (parents of K students received a hard copy of this form in the K roundup packet, 9th grade parents please print form) and have your child's dentist fill it out.
-Return form to your child's building office before October 1st. You may also drop off the completed form at your child's registration location.
**Kindergarten & 9th grade students - A religious exemption may be granted to an applicant if the dental screening conflicts with a genuine and sincere religious belief.
**REQUIRED by the State of Iowa for students entering Kindergarten & 3rd grade.
-The purpose of this vision screening program is to improve the vision and eye health of Iowa's children.
-The Certificate of Vision Screening form is for parents/guardians to have completed by the child's vision screening provider. The vision screening may be completed by an eye doctor, family doctor, community-based organization, child care center or school.
-Fill out a hard copy of this certificate and return to your building office. Form is available at registration locations or you may wish to print it out at home.
**OPTIONAL for K-12 Students
This form was sent in the mail. If you need a new copy to print and fill out you can access it here. Bring printed and completed fee waiver form to your student's building office if you wish to apply for free/reduced lunch.
